CDC Diabetes Prevention Lifestyle Coach Training

The PreventT2 curriculum is based on the original 2002 DPP trial and follow-up studies. It promotes modest weight loss (5-7%) and increased physical activity through a 12 month lifestyle change program and also reflects new literature on self-efficacy, physical activity, and diet. There are 26 modules. In order to achieve CDC recognition, your program must complete at least 22 of these modules.

iHealth Partners Network is a recognized Center for Disease Control and Prevention (CDC) program with credentialed Master Trainers of the National Diabetes Prevention Program. This training provides you with the with the skills, knowledge and experience needed to successfully facilitate and deliver the National DPP lifestyle change program according to the CDC Diabetes Prevention Recognition Program (DPRP) standards with additional course offerings developed by iHPNetwork to furer the promotion and delivery of lifestyle change programs. 
Before you apply, we encourage you to carefully read the  CDC Recognition Program Standards and Operating Procedures to ensure that you have a thorough understanding of the requirements. You should also complete the  Capacity Assessment  to help you decide if your organization has the resources to start and maintain a lifestyle program that meets the requirements for full recognition.

Beyond the Training

CDC requires that organizations be preapproved to offer their curriculum, have lifestyle coaches trained on the CDC Diabetes Prevention Curriculum and have one year of data submitted to continue to be a recognzied CDC Diabetes Program Recognized Program (see CDC Recognition Program Standards and Operating Procedures  - for more information)  The Centers for Disease Control and Prevention (CDC) recognizes iHealth Partners Network for meeting its strict standards and operating procedures.

By being a part of our network and completing the required training you will be immediately qualified to integrate the CDC Diabetes Prevention Curriculum into your current offerings without needing additional approvals, tracking or data collection methods under the iHPNetwork. All the hard work is done for you as a iHPNetwork Member Providers.

Of course you can also become a recognized provider on your own, submit your data manually, find your own referrals - We recommend, that before you move forward on your own, that you complete the  Capacity Assessment  to help you decide if your organization has the resources to start and maintain a lifestyle program that meets the requirements for full recognition.

Once you complete your training, you are elgible to be a recognized provider under the iHPNetwork charter.  By becoming one of our member providers, you can immediatley beging offering the DPP without needing to reapply to the CDC or worry about data collection since our it's automatically built-in to our care coordination platform.
Many health plans, self-insured organizations and other third-party payers are beginning to reimburse for diabetes prevention programs that are recognized by the CDC National Diabetes Prevention Program.

We submit the data for you as a member of our network.  
 Medicare is also looking at reimbursing recognized programs that offer the CDC Diabetes Prevention Program Curriculum beginning January 2018 ... but you must demonstrate your programs succss for a year with auditable data and be recognized to offer the CDC Diabetes Prevention Program.
