Have Prediabetes

84.1 million Americans have prediabetes, a condition where blood sugar is higher than normal, but not high enough to constitute a type 2 diabetes diagnosis. 15-20 percent of those individuals will develop type 2 diabetes within 5 years, leading to increased healthcare spending on diabetes management. However, if caught early through screening, prediabetes is reversible through lifestyle changes.

The Medicare Diabetes Prevention Program (MDPP) expanded model is a life-long structured behavior change intervention that aims to prevent the onset of type 2 diabetes among Medicare beneficiaries with an indication of prediabetes and is covered for the first 3-years.

What is prediabetes?

Prediabetes is a condition in which blood sugar, whether measured as HbA1c, a test that measures your blood glucose over a 2-3 month timespan, or glucose, is higher than normal, but not high enough to be considered type 2 diabetes. The American Diabetes Association defines diabetes as HbA1c measuring 6.5% or higher, while an HbA1c result of 5.7% to 6.4% means a person has prediabetes. A fasting glucose measurement of 100 mg/DL to 125 mg/dL is also considered prediabetes.

Today, the CDC estimates that one third of the American adult population has prediabetes. There are no clear symptoms, which is why nearly 90% of individuals with prediabetes do not know they have the condition. However, certain risk factors mean a person may have a predisposition for prediabetes.

These include:
  • Being overweight or obese
  • Having a family history of diabetes
  • Being physically active less than 3 times per week
  • Having a history of gestational diabetes
  • Being of minority descent (including African American, Hispanic, American Indian, or Asian American)

Eligible beneficiaries are those who:

• Are enrolled in Medicare Part B (No Co-Pay to Participants...EVER)

• Have a body mass index (BMI) of at least 25, or at least 23 if self-identified as Asian

• Meet 1 of the following 3 blood test requirements within the 12 months of the first core
  1.  A hemoglobin A1c test with a value between 5.7 and 6.4% , or
  2. A fasting plasma glucose of 110-125 mg/dL, or
  3. A 2-hour plasma glucose of 140-199 mg/dL (oral glucose tolerance test)

•Have no previous diagnosis of type 1 or type 2 diabetes (other than gestational diabetes)

• Do not have end-stage renal disease (ESRD)

How it Works

  1. Biometric Screening
    Whether residents use their own primary care physician or attend a wellness screening as an optional low cost fee-based service through iHealth Partners Network at your location, completing a screening will give your residents a new perspective on their health numbers. (Blood pressure, BMI, Glucose, HDL/LDL Cholesterol levels)
  2. A Health Coach
    A lifestyle coach, specially trained to lead the program, to help your residents learn new skills, encourage them to set and meet goals, and keep them motivated. The coach will also facilitate discussions and help make the program fun and engaging.
  3. Health Education
    Each week, participants receive CDC-approved curriculum with lessons, handouts, and other resources to help your residents make healthy changes.
  4. Tools
    All participants have access to our smart app with connects to their health coach, physician and any other care provider they choose. In addition, wireless scales and wearable devices can also be connected (free) so participants can track their progress, and reveal what is and isn’t working.
  5. On-Going Support
    A support group of people with similar goals and challenges. Together, participants can share ideas, celebrate successes, and work to overcome obstacles. In some programs, the participants stay in touch with each other during the week. It may be easier to make changes when working as a group than doing it on their own.
  6. Clinicians Never Lose Track of Patients
    iHealth Partners Network ensures that all your residents' primary care providers are kept in the loop beyond simple follow-up communications. Our process includes the physician in all communications and updates via our unique care platform physicians never lose track of their patients.

What Your Residents Will Learn in the First Year

During the first half of the program, they will learn to:  
  • Eat healthy without giving up all the foods you love
  • Add physical activity to your life, even if you don’t think you have time
  • Deal with stress
  • Cope with challenges that can derail your hard work—like how to choose healthy food when eating out
  • Get back on track if you stray from your plan—because everyone slips now and then

In the second half of the program, they will enhance the skills already learned so to maintain the changes made. These sessions will review key ideas such as:
  • tracking your food and physical activity,
  • setting goals, staying motivated, and overcoming barriers.
  • They may learn some new information, too.
The lifestyle coach and small group will continue to support you.
